Lately I've had a bunch of people asking me about the Dame title, how to get it, and whats all the buzz about the Dame and Sir prefix titles being bugged? I can't tell you how many times this week people have asked me if the Sir prefix achievement line is bugged!

Also keep in mind that the Dame title is only for female characters, and the Sir title is for male characters. The same achievement line rewards one or the other. You cannot have both Sir and Dame prefix titles.
